This form is for use on match days.

Only TWO players can be signed at a match using this procedure, it cannot be used in SVFL Cup competitions.
All players being signed by this method must present to the opposing team official a valid photo ID which clearly states their date of birth.

A drivers licence or passport.

If no ID of this type is presented then the registration MUST not be permitted.

The players details are to be input into the Whole Game System within 48 hours of the match in which they played and the form with a copy of the Photo ID is to be sent to the league registrations secretary John Forbes to reach him within 7 days of the match.
